Indonesia is set to blame plane design and oversight failures for the fatal crash of a Boeing 737 Max jet in October, The Wall Street Journal reported Sunday.

People familiar with the investigation told the outlet that the draft conclusions also identified pilot errors and maintenance mistakes as causal factors involved in the deadly accident.

{mosads}Misfires of an automated flight-control feature called MCAS on the Boeing plane led to the nosedive of the Lion Air jet and a similar crash of an Ethiopian Airlines flight.

The Boeing 737 Max has been grounded since those two crashes, which left a combined 346 dead.

Details of the Indonesian report are subject to change and further analysis, according to the Journal.

Once released, the report will likely be the first official government finding of fault in the crashes.

U.S. regulators are expected to make a set of safety recommendations for Boeing later this month, according to the Journal.
